This task can also be performed from the Study and Cancer Group Administration module. See Viewing a Previously Uploaded Consent Form.

Consent forms that have been uploaded into the CCR database are stored in pdf file format. In order to view this file format you must have a pdf reader application, such as Adobe Acrobat Reader, installed on your computer. This application is free and is easily accessible for download on the Internet. If you do not have a pdf reader on your computer, contact HCI Informatics for help in acquiring the proper software.

To view a previously uploaded consent form:

Make certain that if you have a pop-up blocker installed on your computer it is either turned off, configured to accept pop-ups from the HCI server or that you hold down the proper key(s) — usually the CTRL key — to allow the pop-up window to open while doing the following steps. The consent form will appear in a pop-up window, so a pop-up blocker will interfere with this functionality. (If you don't know if you have a pop-up blocker installed on your computer, don't worry about it unless you run into trouble.)

Open the Study Enrollment window for the patient's enrollment in the study. See Viewing/Editing a Patient's Enrollment in a Study from the Patient Detail Window for instructions on how to do this.

Double-click on the consent form that you want to view in the Uploaded Consent Forms grid on the window (identified by the date on which it was uploaded). A separate browser window (outside of the CCR desktop environment) will open displaying the pdf file associated with the row in the Uploaded Consent Forms grid that you selected.
