The CCR Patient Review page is designed for use during a clinic visit, giving easy-to-find access to the kind of information a medical provider might need during a clinic visit but also providing for easy data entry into CCR of information that might typically be gathered during a clinic visit. The idea is that this will both improve the quality and efficiency of clinic visits while helping to facilitate the timely entry of relevant information into CCR to better support both clinical and research uses of the database. Among data which could be entered in this fashion are treatment protocols assigned to a patient, diagnoses and staging results. To add a diagnosis to a patient record from the CCR Patient Review page:

1. Open the CCR Patient Review page for the patient in question — see Viewing a Patient's Clinic Visit Patient Review Page for instructions on how to do this. Show me the CCR Patient Review page.

2. Click on the Add Diagnosis button near the top of the page. The Diagnosis window will open.

3. Enter the diagnosis information on the Diagnosis window. For a description of the window and the various fields available to enter information, see The Diagnosis Window. Click the Save button to save the diagnosis and add it to the patient record. The window will close and the newly entered diagnosis will be added to the patient record.

To close the window without adding a diagnosis to the patient record, click the Cancel button. The window will close without saving any of your changes.

As with diagnoses entered using the patient detail window, the cancer diagnosis you enter is associated with your cancer group as specified in your session purpose. Users from other cancer groups viewing the patient record will not see the diagnosis in the grid unless they have marked the Show Diagnoses And Stages For All Cancer Groups checkbox above the Diagnosis grid on the patient detail window — see The Overview Tab (Patient Detail Window).
