Diagnoses can also be added to a patient record from the CCR Patient Review page. See Viewing a Patient's Clinic Visit Patient Review Page, The CCR Patient Review Page and Adding a Diagnosis to a Patient Record From the CCR Patient Review Page for more information about adding a diagnosis record using this alternative method.

To add a diagnosis to a patient record:

Open the patient detail record for the patient in question. See Viewing/Editing a Patient Record for instructions on how to do this. Make sure the Overview tab is selected.

Click on the + button above the Diagnosis grid on that tab. Show me the Diagnosis grid. Show me the button. The Diagnosis Detail window will open.

Enter the diagnosis information on the Diagnosis Detail window. For a description of the window and the various fields available to enter information, see The Diagnosis Detail Window.

Click the Save & Close button to save the diagnosis and add it to the patient record. The window will close and the newly entered diagnosis will be appear in the Diagnosis grid on the patient detail window.

To close the window without adding a diagnosis to the patient record, click the Cancel button. If you have entered information on the window, a dialog will open asking you to confirm that you want to close the window without saving your changes. Click Yes to continue to close the window or No to return to the Diagnosis Detail window.

As soon as you start entering data on the Diagnosis Detail window, the text on the Save button will turn bold red, indicating the need to click on the button to save your work. To save your work so far without closing the window, click on that Save button.

The cancer diagnosis you enter is associated with your cancer group as specified in your session purpose. Users from other cancer groups viewing the patient record will not see the diagnosis in the grid unless they have marked the Show Diagnoses And Stages For All Cancer Groups checkbox above the grid — see The Overview Tab (Patient Detail Window).
