Pain Medicine and Palliative Care Center (PMPC) incorporates two online patient questionnaires:

  1. a New Patient Questionnaire,
  2. and a Follow-up Symptom Questionnaire.

The online system is driven almost directly from the medial event forms which Cancer Clinical Research (CCR) cancer groups are familiar. The on-line survey is custom coded for special handling (skipping questions depending on the response, formatting, etc).

PMPC Symptom Questionnaire comes with a kiosk console for patient input and administrator's console for security, patient registration and management. Online patient entry ensures accuracy.

Key Features include:

  • Cancer Group Management – Easily managed by each cancer group. Privileged users are able to view and manage the survey data.
  • Ease of Creation – Each survey is created as a medical event within CCR.
  • Configurable Behaviors – The on-line surveys can be custom coded for special handling (skipping questions depending on the response, formatting, etc) and allows for tabular or single page display.
  • Provides option for completeness and will not allow patient to submit until required questions, which are highlighted, are answered.
  • Ability is provided to turn scrolling on and off and set colors.
  • Clinical/Research Security Model – Provides a robust security model that allows for separation and protection of rights and privileges of the Administrator from the Patient's completion of the survey.
  • The Time-Out feature allows the administrator to set the length of time the patient kiosk can remain idle before it is shut down.
